Our Outpatient Geriatric Assessment Program can be an important resource for seniors and caregivers to help identify medical concerns and health care needs. Our uniquely qualified, board-certified geriatrician (a doctor who specializes in geriatric medicine) and geriatric team will conduct a thorough evaluation to help you better understand and manage the physical and mental challenges associated with aging.

An in-depth, geriatric assessment gives doctors an overall picture of the senior's health status-physical condition and psychological health (including memory loss, social well-being and support networks).

  Our comprehensive physical assessment includes a detailed review of all medications and possible drug interactions, an eye exam and a hearing assessment with referrals for further evaluations, when appropriate. Our geriatric team works closely with each patient's primary care physician to consult on recommended approaches and to coordinate care and services.

  We recognize that aging impacts more than just the body. Over the course of several appointments our geriatric specialists will take the time to get to know the senior patient and his or her caregiver. That way, team members can make appropriate recommendations for any medical referrals or support services needed.
