Acute Care of the Elderly - The ACE Unit

Our mission is to provide senior patients with the highest quality care and services that offer comfort, promote wellness, preserve dignity and foster as much independence as possible during their hospitalization.

What is an ACE unit?
It is a model of inpatient care for the older adult that seeks to avoid complications and maintain the level of baseline function prior to hospitalization. It is targeted to improving clinical outcomes in the elderly. Patients who are 65 years of age and older make up 40-50% of hospital admissions and about half of all hospital days.

What makes an ACE unit different?

  • An interdisciplinary approach with trained geriatric staff.
  • The team meets on a scheduled basis to conduct rounds and review each patient.
  • The team works to integrate care planning and improve continuity of care and communication with patients and families.

ACE Goals

  • To care for the frail and/or, cognitively impaired, patients composed primarily of those living in extended care or assisted living facilities.
  • To care for patients who require acute care for medical needs: evaluation, stabilization and treatment with the goal of returning to their previous setting and minimizing the complications associated with geriatric syndromes.
    • Return patient to extended care or assisted living facility within a designated timeframe.
    • Decrease length of stay.
    • Reduce readmission rate.
    • Prevent and/or minimize the following:
      • Falls •Skin breakdown
      • Restraint usage
      • Delirium
      • Cognitive decline
      • Immobility
      • Constipation
      • Foley catheter usage
