Home Health Care
Christiana Care Visiting Nurse Association  specializes in caring for people where they are most comfortable - at home. Services include geriatric nursing, nutritional sup-port and counseling, certified home health assistance (personal care, light housekeeping, companionship and caregiver respite), rehabilitation therapy, in-home disease management, Alzheimer's day treatment, social work, specialty care, home infusion services, home medical equipment and medical and surgical supplies.

Rehabilitation
Christiana Care offers physical, speech-language, swallowing and occupational rehabilitation services in a variety of settings for both inpatients and outpatients of all ages. Our Center for Rehabilitation  at Wilmington Hospital is fully accredited and specializes in helping survivors of stroke, fractures, brain injuries, neurological disorders, joint replacements, myopathies, cancer, amputations and other disabling illnesses or injuries.

A variety of outpatient services and programs are offered at both Wilmington and Christiana hospitals and through a network of physical, occupational and speech-language rehabilitation services at several convenient Physical Therapy PLUS  locations. In-home services are provided by Christiana Care Visiting Nurse Association. Medically supervised cardiac rehabilitation services are also offered at the Preventive Medicine & Rehabilitation Institute and the Healthcare Center, Christiana.

Senior Health Services

Balance and Mobility
Balance problems, often marked by dizziness, can be caused by a wide range of physical conditions and can be particularly worrisome for seniors and their loved ones who worry about the risk of falls. The Balance and Mobility Center  at Wilmington Hospital offers help for people with chronic balance and movement disorders and the hope to improve their quality of life.

Congestive Heart Failure
Seniors diagnosed with heart failure can rely on their very own cardiac nurse for advice and support. Christiana Care's Heart Failure Education Program offers the assurance of knowing there's a specially trained nurse just a phone call away to answer questions or concerns, monitor health status, reinforce doctor's instructions and explain medications. The program helps those with heart failure develop a plan to recognize warning signs, reduce troublesome symptoms and avoid re-hospitalization or trips to the emergency room.

Depression
Emotional pain has many faces. Help is available through the Pathways Assessment and Referral Service, an important link to both hospital and community resources. Pathways is part of Christiana Care Psychiatric Services , which offers confidential and comprehensive evaluation and treatment options, including medication, individual and group psychotherapy, family therapy, crisis intervention and support services.

Diabetes
Christiana Care's Diabetes Education Team  offers the specialized attention seniors might need to keep their diabetes under control. Team members, including registered nurses, dietitians and pharmacists, assess patients' abilities to manage their diabetes and provide training to both seniors and caregivers for meal planning, blood sugar monitoring, prevention and correction of low blood sugar, understanding medications and what to do on sick days.

Exercise for Seniors
Regular exercise is important for seniors who want to remain active and independent well into their 'golden years.' What's more, exercise-with their doctors' approval -can help people suffering from chronic illness reduce symptoms and improve daily performance. Christiana Care's Preventive Medicine & Rehabilitation Institute  offers a variety of exercise programs and individualized instruction for seniors, including low-impact aerobics, balance training, cardiac rehabilitation, post-physical therapy conditioning, yoga, Tai Chi and resistance training. Our Health Through Fitness Program offers the guidance and safety of medically supervised exercise for people at risk or diagnosed with a chronic condition.

Senior Services is your connection to senior-sensitive care. All services are coordinated with the patient's primary care doctor, and some require a doctor's referral to participate.

Falls Prevention
For people over 65, most accidental falls occur in the home. The consequences are potentially devastating, but preventable. Christiana Care offers in-home assessments by trained physical and occupational therapists through the Balance and Mobility Center. Christiana Care Visiting Nurse Association  also helps provide a 'fall-safe' home environment to reduce the likelihood of falls.

Hearing Services
Hearing loss associated with aging can isolate loved ones from social contacts and lead to withdrawal from the outside world. Christiana Care offers screening and diagnostic hearing tests performed by clinical audiologists who can identify hearing problems and suggest corrective measures. They can help you find the right hearing aid or choose from a variety of assisted-listening devices.

Knee and Hip Replacements
A fractured hip or joint injury can turn life upside down in seconds. When surgical treatment is required for chronic joint pain or traumatic injury, the Center for Advanced Joint Replacement  at Wilmington Hospital can set the world right again. Our patients enjoy the Center's 'non-hospital' atmosphere and benefit from effective, contemporary surgical approaches to relieve symptoms and restore strength and function. Physical and occupational therapy, education and counseling services, and home care options are all part of the Center's special brand of orthopedic medicine.

Osteoporosis
For many women over 65 (and some men), the debilitating side effects of osteoporosis (spinal fractures, pain, deformity and loss of mobility) can shrink the world around them to the four corners of a sitting room. Christiana Care offers screening, education and exercise programs designed to help people protect aging bones from osteoporosis and its long-term, serious consequences.

Urinary Continence
Incontinence need not interfere with doing the things in life that matter most. Christiana Care's Urinary Continence Program offers the latest techniques to assess bladder problems and develop a treatment plan.

Vision Care
Regular eye care can help ensure a lifetime of good vision. The Wilmington Hospital Health Center offers a full range of vision care services for seniors, including routine vision screenings and treatment for eye problems such as glaucoma, retinal diseases and cataracts. We offer the latest in preventive and corrective eye care that includes laser treatments, retinal and cataract surgery and corneal transplants. Outpatient surgeries are scheduled at either Wilmington Hospital or the Roxana Cannon Arsht Surgicenter on the Wilmington Hospital campus.

Wound Care
Christiana Care's Wound Care Centers  at Riverside and in Middletown offer the latest treatments for patients with chronic, non-healing wounds, including those associated with poor circulation, abnormal blood pressure and diabetes. We're part of a national network of 170 Wound Care Centers affiliated with Curative Health Services.
