Flexible Benefits Package

Benefit Eligibility Description
Medical Care

1st of the month

Two (2) options are available for medical coverage. 

  • Core Provider Option - PPO
    (Blue Cross/Blue Shield of Delaware)
  • Preferred Provider Option - PPO
    (Blue Cross/Blue Shield of Delaware)

Medical Care coverage for both PPO Plans, requires employee contributions for all coverage levels.

Contributions for part-time employees are pro-rated based on employee scheduled/budgeted hours.

Prescription Card (Rx)

The Rx benefit is offered as part of both medical plan options. Express Scripts administers the prescription card program.

Formulary: The plan utilizes a listing of preferred drugs with lower copays. Generic and Branded Rx drugs are covered.

Dental Plan

Administered by Delta Dental
Requires contribution for all coverage levels.

  • Annual deductible:  $50/Single  - $150/Family. (Deductible does not apply to preventive care services)
  • Plan covers the diagnostic and preventive care services, including exams, X-rays & prophylaxis at 100% of the Allowed Amount with a Delta dentist based on the service provided.
  • Basic restorative services: including endodontics, extractions, fillings, periodontics & anesthesia paid at 75% of Allowed Amount
  • Major restorative services including: crowns, inlays and onlays paid at 75% of Allowed Amount
  • Prosthodontia services: including bridges and dentures paid at 75% of Allowed Amount
  • $1,500 per person annual plan year maximum
  • Limited Orthodontic Services offered to dependents only up to age 19, Lifetime maximum $1,000

Contributions for part-time employees are pro-rated based on employee scheduled/budgeted hours.

FSA: Health Care and Dependent Care Accounts

1st of the month following start date

  • Administered by SHPS
  • FSA maximum employee annual account contribution
    • Health Care account:  $5,000 maximum deposit
    • Dependent Care account:  $5,000 maximum deposit
  • Deposits set aside with before tax dollars for payment of health care and dependent care expenses
  • Health Care Account covers certain out-of-pocket health care or dependent care expenses to pay with before tax dollars
  • Dependent Care Account covers child care expenses for children age 13 and under
Long Term Disability (LTD)

1st of the month following start date

Full-time employees provided benefit by Christiana Care at no cost to employee:

  • 50% Salary Replacement - 90 day wait
  • Maximum monthly benefit - $10,000

Enhanced options available with additional employee contribution

  • 65% Salary Replacement - 90 day wait
  • 65% Salary Replacement - 180 day wait

Part-timeemployeesin budgeted positions with work schedules of 30 or more hours a week (at least 60 hrs. per pay period) may also enroll in the above benefits for a monthly premium

Group Life Insurance

1st of the month following start date

  • Employer provided: 1 times annualized base pay, at no cost to employee
  • Optional Coverage: employee may choose an additional 1, 2, or 3 times base pay at additional cost based on age
  • Maximum insurance up to the lesser of 4 times base salary (includes optional) or $1 million dollars
  • An insurance amount in excess of $750,000 is subject to Evidence of Insurability (EOI)
Personal Accident Insurance (PAI)

1st of the month following start date

Employee: 6 coverage options – employee purchase, 100% contributory

  • Employee Coverage: $25,000, $50,000, $100,000, $150,000, $200,000 & $250,000

Family Coverage: Based on Employee PAI Amount Selected to an overall family maximum benefit of $50,000:

  • Spouse only - 60% of employee amount
  • Spouse with children - Spouse 50%, Children 15%
  • Child/ren only 20% up to age 18

Professional Development

Tuition Assistance Plan

  • Covers Associate’s, Bachelor’s, or Master’s degree

Scholarships

  • Program covers tuition and text books for students in nursing and selected Allied Health programs

Paid Time Off

  • 26 to 36 days per year, based on position and length of service
  • Varies depending on vacation time allotted to specific positions and length of service
  • Maximum balance equals 2x yearly accrual
Disability Leave Account

After completion of 90 days of service

  • Accrual starts from date of hire, minimum  5 days per year maximum based on position and length of service
  • Paid time away from work for extended illness or leave of absence
  • Used for absences due to illness after 24th consecutive scheduled work hour of absence

Employee Assistance Program (EAP)

Immediate

To help with work/life balance and provide assistance whenever life gets challenging, the Christiana Care Employee Assistance Program (EAP)   is a confidential resource for assistance with a wide range of concerns and needs, including stress, relationship problems, family matters, financial concerns, emotional distress and many other issues. The benefits are prepaid by Christiana Care and offered at no cost to employees.

This free benefit is provided by ComPsych insurance company. Support is available 24 hours a day, 7 days a week, by phone or on the web. This program provides unlimited telephone consultations with life balance consultants, three free visits with a local EAP provider, web-based tools and online information for you, your family and/or your dependents.

Life balance professional staff consultants can provide one-on-one assistance for a variety of life issues including:

  • Up to 5 face-to-face sessions per issue with a licensed professional EAP counselor close to work or home
  • Coping with stress, anxiety and depression
  • Caring for an elderly relative or finding senior assistance
  • Communication difficulties with a manager or co-worker
  • Raising children and finding child care in your area
  • Dealing with a family member who has an addiction
  • Improving your health or controlling a chronic condition
  • Getting out of debt and managing your finances
  • Relationship issues, legal issues and much more

Other Programs that support work-life balance include:

  • Flexible work options including – flex time that varies starting time and is available based on departmental function, part time with benefits, part time with limited benefits for 15% higher pay, compressed work weeks with 10 and 12 hour shifts, Weekend Incentive Program that provides higher levels of pay for 3 out of 4 weekends, casual /per diem, job sharing, telecommuting
  • Leaves of absences
  • Preventive Health Services: on-site services that provide education, prevention and screening (breast cancer, cholesterol, blood pressure, etc.) tools as well as health information, including recipes and other tips. Free flu shots are given on an annual basis
