Care Management Team
When you are facing the challenge of cancer, finding the right care and services you need can seem overwhelming. Your nurse navigator from the Helen F. Graham Cancer Center's cancer care-management team works with you and your doctor to help plan the best care for you, both in and out of the hospital.
The main function of your cancer-management team is to coordinate the plan of care, including inpatient and outpatient services, that your doctors, you and your family have outlined. Our objective is to provide high-quality, patient-centered care that focuses completely on you.
The Cancer Care Management team works with the entire team of cancer professionals, including physicians, nurses, social workers, dietitians, chaplains, cancer researchers, the oncology data center, librarians, health psychologists, complementary-medicine providers and community-resource providers.
You will be assigned a nurse navigator and a social-work team. This team will follow you during your entire course of treatment. The nurse navigator acts as the team leader and provides support, education and a comprehensive assessment. This assessment of need triggers referrals to other team members, to resources within the community and to services at the Helen F. Graham Cancer Center.
The Cancer Care Management Department:
- Provides individual and support services to patients and their families.
- Coordinates elements of care.
- Assists with communication to ensure that your doctors have all the necessary information to diagnose and provide you with effective treatment in a timely manner.

Helps you to learn about any clinical trials for which you may be eligible.
