Home Doctor Visits

Patients who cannot leave their homes because of illness or disability can get the medical care they need from the staff at Christiana Care's Family Medicine. Our home-visit team of doctors, nurse practitioners and social workers is available to make house calls to homebound patients who live in Wilmington and New Castle.

Our home-visit team provides:

  • Continuous, coordinated medical care between home and the hospital.
  • Intimate knowledge of the home environment and care givers.
  • Better control of chronic diseases.
  • Identification of problems for early intervention.
  • Careful management of acute care.
  • Coordination of community-based services.
  • Humane and compassionate end-of-life care.
  • A close link between community elderly and the health care system.

Our team can be available for acute problems within 24 to 48 hours. We will also schedule regular visits for ongoing care.
